Make Your Own Forms
on the iPad with Numbers, by Jim & Kathi Lengel and Molly Dubois

Objectives

Getting Started
1. Launch the Numbers app.
2. Create a new blank spreadsheet.
3. Enter a title for this form, In cell A1. (You'll need to double-tap the cell to open up the editing panel.)
4. In cell A2, enter your first label, such as 'Teacher's name:'
5. Enter other labels as necessary in adjacent cells.
6. Enter your first item in the next cell below, such as 'Keeps students occupied with academic tasks on their iPads.'
7. Expand the width of this column to make the item easier to read. (To do this, tap the handle over the top of the column, then drag the || to widen the column.)
8. To enter a rating scale for an item, double tap the cell to the right. This will open an editing window.
9. In the editing window, tap the stars in the lower left.
10. To enter a checkbox, click the checkbox in the lower left.
11. Enter additional items, rating scales, and checkboxes as necessary.
12. To compute an average rating, double tap the cell below the last rating scale. This will open an editing window.
13. In the editing window, tap the equal sign = just to the left of the cursor.
14. Now tap the function button on the right.
15. Tap the AVERAGE.
16. Tap and drag over the cells you want to average.
17. Click the green checkmark on the right.
18. You will see Numbers compute the average.

To use your new form...
1. Click the Spreadsheets button in the upper left.
2. Find your new spreadsheet on the page.
3. Tap and hold it until its selected.
4. Tap the duplicate button. Watch the copy add itself to the page.
5. Use this copy to complete the form.
6. Repeat steps 3 to 5 to make another copy of the form.

To send a form to someone else...
1. Launch Numbers and open the form.
2. Tap the wrench icon in the upper right.
3. Tap 'Share and Print.'
4. Tap 'Email spreadsheet.'
5. Enter the email address of your correspondent.
6. Send the mail.
